Summary:

Initiates, facilitates, and performs a variety of casework related functions pertaining to the development and implementation of the integrated, person - centered plan which encompasses all aspects of the participant’s life. The Case Manager/QIDP is responsible for ensuring residents receive direct and indirect support across every aspect of their daily lives including dignity, self-advocacy, and increased autonomy. The Case Manager/QIDP is responsible for direct oversight and case management of family/individual cases and providing in home support coverage as necessary.

Reports to: CDS Director of Case Management

Principle Duties / Responsibilities:

· Carries an average caseload of up to 50 adults receiving Community Day Services in a variety of potential living arrangements.

· The primary internal, service provider advocate for participants on their caseload in compliance with agency mission, program guidelines, best practices for individuals with ID/DD,

· Coordinates and leads the multidisciplinary team through the collaboration process to meet the needs of individual’s served.

· Maintains accountability for the upkeep of participant files on their caseload and ensures the delivery and/or coordination of all Community Day Services.

Requirements:

· Required bachelor’s degree in a human service field.

· Must be Qualified Intellectual Disabilities Professional (QIDP) eligible or certified.

· Required 1 year of Professional Direct Care experience working with adults/children who have intellectual / developmental disabilities.

• Staff are expected to have the ability to work remotely from home if necessary, and with Supervisory approval. Safety Protocols while at the office and within the community are to be strictly adhered.

Mental/Physical Demands:

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Work requires prolonged concentration using a computer in analyzing and manipulating data. While performing the duties of this job, the employee is regularly required to use hands to finger, handle or feel and talk or hear. The employee frequently is required to sit. The employee is occasionally required to stand and walk. The employee must lift and/or move up to 10 pounds and occasionally life and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and ability to adjust focus.

Environmental Conditions: The noise level in the work environment is usually moderate. Duties are performed in a typical office environment. No unusual or adverse working conditions exist.
