To further our mission, we are looking for a Case Manager. The Case Manager is responsible for providing intensive case management services to help guide clients to achieve self-sufficiency and permanent housing.
ESSENTIAL FUNCTIONS
- Document activities and interactions with clients through detailed written case notes.
- Provide case management services for areas including school enrollment, employment, and referrals to other services as needed.
- Secure housing for program participants by visiting apartments throughout the Pittsburgh area ensuring their cleanliness and safety.
- Teach and model independent living skills.
- Develop, monitor, and revise client goal plans.
- Meet with clients as appropriate to ensure that goal plans are being achieved.
- Conduct monthly unit inspections.
- Act as a liaison between client and landlord.
- Develop discharge plans to include a two-month aftercare plan.
- Maintain and update all clients’ files per DHS regulations.
- Coordinate activities in the moving of participants into their apartments.
- Performs other duties as assigned.
EDUCATION AND EXPERIENCE
- High school diploma/equivalent required, Bachelor’s degree preferred.
- Two years of experience in a social services field required.
REQUIRED SKILLS/ABILITIES
- Develop and maintain positive relationships with clients and team members.
- Maintaining a physically and emotionally safe environment.
- Ensure that program rules and expectations are carried out consistently.
- Ability to relate to young people from diverse backgrounds.
WORK ENVIRONMENT AND PHYSICAL DEMANDS
- General office environment.
- Local travel required.
- Some evening and weekend hours.
- While performing the duties of this job, the employee is regularly required to use a computer for extended periods of time and be able to communicate using a computer and phone/smart device.
- The employee frequently is required to move about the work, apartment, and outside environment.
- The employee must occasionally lift and/or move up to 10 pounds.
