Job Summary
My Place FFI provides transitional housing for foster youth ages 18 and older who remain involved with CYF. Participants work closely with an FFI Case Manager and receive intensive case management focused on building self‑sufficiency and securing permanent housing.
Essential Functions
- Develop and maintain positive relationships with clients in the program.
- Teach and model independent living skills.
- Provide case management duties regarding school, employment and referrals:
- Maintain and update all clients’ files per CYF regulations
- Document activities of casework and interaction with clients through written case notes
- Cultivate a physically and emotionally safe environment.
- Meet with clients at program office.
- Attend and participate in monthly life skills classes with My Place staff and clients.
- Help in securing housing for program participants by visiting apartments throughout the Pittsburgh area and ensuring their cleanliness and safety.
- Complete initial assessments for new clients within required program time frames and update them as client information changes.
- Develop, monitor, and revise the individual goal plans in conjunction with client.
- Collaborate with clients to develop a discharge plan to include provision of two-month aftercare plan services for program graduates.
- Submit the program paperwork folder to the Supervisor every Monday by 12pm.
- Establish and maintain relationships with ILP agencies in Allegheny County, and other service providers as needed.
- Prepare all reports as required by funding agencies.
- Other responsibilities as assigned.
- Degree in Social Services, or 3 to 5 years of experience working with youth required.
- General office environment.
- Local travel required.
- Physical strength to manipulate light to medium weight materials weighing 10 to 15 pounds on a regular basis.
