Job Description
This position is based in a community clinic, focusing on case management. The role involves completing initial and 180-day assessments for participants. You will be highly involved in hospital discharges, collaborating with hospital discharge planners to ensure a safe and appropriate discharge plan, while effectively communicating recommendations. You will also coordinate Skilled Nursing Facility (SNF) discharges for participants discharged from the hospital or community for rehabilitation or skilled needs. Working within an Interdisciplinary Team (IDT) is essential to ensure continuity of care for the participants you oversee.
Responsibilities
- Conduct initial and 180-day assessments for participants.
- Collaborate with hospital discharge planners for safe and appropriate discharge planning.
- Coordinate Skilled Nursing Facility (SNF) discharges for participants requiring rehabilitation or skilled needs.
- Work within an Interdisciplinary Team (IDT) to ensure continuity of care for participants.
Essential Skills
- 1 years of experience in RN Case Management.
- Proficiency in EPIC, utilization review, appeals, care coordination, and discharge planning.
- Registered Nurse in Alaska
Additional Skills & Qualifications
- Basic Life Support (BLS) certification.
Work Environment
The position is based in an outpatient facility with short-term acute care. You will work in a professional healthcare setting, collaborating with various healthcare professionals to ensure optimal care for participants.
Job Type & LocationThis is a Contract to Hire position based out of Anchorage, AK.
Pay and BenefitsThe pay range for this position is $35.00 - $45.00/hr.
