Job Description
We are seeking a dedicated Pharmacy Technician to join our population health team in Texas. This role is remote and focuses on supporting medication reconciliation processes, optimizing medication regimens, and ensuring compliance with regulatory standards. The ideal candidate will work closely with healthcare teams and contribute to initiatives aimed at improving health outcomes and reducing medication-related disparities.
Responsibilities
- Support medication reconciliation processes for patients within the designated population.
- Collaborate with pharmacists and healthcare providers to optimize medication regimens.
- Review patient profiles to identify potential medication-related issues and discrepancies.
- Collect and analyze medication-related data to identify trends and opportunities for improvement.
- Generate reports on medication adherence, therapeutic outcomes, and other relevant metrics.
- Work closely with healthcare teams to ensure seamless coordination of pharmaceutical services.
- Communicate with patients, caregivers, and healthcare professionals to address medication-related concerns.
- Participate in population health programs aimed at promoting medication adherence and preventive care.
- Support initiatives focused on improving health outcomes and reducing medication-related disparities.
- Ensure compliance with regulatory standards, pharmacy laws, and organizational policies.
- Contribute to quality improvement efforts within the pharmacy setting.
Additional Skills & Qualifications
- High school diploma or equivalent; completion of a pharmacy technician program preferred.
- Texas state board of pharmacy, TSBP certification.

Pay and BenefitsThe pay range for this position is $21.00 - $21.00/hr.
