The pharmacist delivers expert education on the safe and effective use of intravenous and oral chemotherapy and supportive care medications, ensures accurate and timely processing of treatment orders, and proactively identifies, resolves, and prevents medication-related problems in the hematology/oncology population. The position also contributes to regulatory compliance, financial considerations related to chemotherapy, and may perform broader pharmacy department duties with appropriate competencies.

Responsibilities

  • Act as the primary pharmacy liaison to the Hematology/Oncology Clinic and Cancer Center Infusion, ensuring seamless communication and collaboration across the care team.
  • Be readily available to providers and nursing staff for consultation on oral and intravenous chemotherapy and supportive care medications, including dosing, interactions, and monitoring parameters.
  • Provide expertise and support for electronic health record treatment plans, including building, reviewing, and optimizing chemotherapy regimens within the system for the Cancer Center team.
  • Deliver individualized counseling to patients and families on chemotherapy regimens, emphasizing medication adherence, management of adverse effects, and safe handling of chemotherapy and supportive care medications.
  • Verify medication orders and provide comprehensive clinical pharmacy services to nursing staff, providers, and patients, including floor or unit rotations, ongoing patient monitoring, admission medication history review, value monitoring, and active participation on the code team.
  • Directly supervise pharmacy interns, technicians, and students in their daily work activities, including dispensing, sterile product preparation, inventory control, repackaging, and providing the professional check for these processes.
  • Maintain responsibility for the control, dispensing, and maintenance of all medications used within the hospital, and perform or assist with departmental tasks associated with these procedures.
  • Participate in departmental and systemwide quality assurance and performance improvement programs, contributing to initiatives that enhance medication safety and clinical outcomes.
  • Support regulatory compliance related to chemotherapy and pharmacy operations, including documentation, process adherence, and readiness for inspections or audits.
  • Collect and evaluate data related to medication events, adverse drug reactions, quality assurance activities, and drug utilization review, and use findings to recommend and implement improvements in medication use processes.
  • Assist department leadership with chemotherapy-related regulatory and financial considerations, including cost-effective medication use and optimization of treatment protocols.
  • Perform pharmacist duties related to chemotherapy dispensing and other staff pharmacist responsibilities within the Pharmacy Department, once appropriate competencies are completed and maintained.

Essential Skills

  • Active or eligible pharmacist license, with the ability and willingness to obtain a Wisconsin Pharmacist License if currently licensed in another state.
  • At least 1 year of experience working in an Oncology and Hematology clinic or similar hematology/oncology practice setting.
  • Demonstrated clinical expertise in oncology and hematology pharmacotherapy, including chemotherapy regimens and supportive care medications.
  • Proficiency with intravenous compounding and chemotherapy preparation in accordance with safety and sterility standards.
  • Strong ability to identify, solve, and prevent medication-related problems in complex oncology and hematology patient populations.
  • Experience using electronic health record systems for treatment plan management, order entry, and clinical documentation.
  • Capability to verify medication orders accurately and provide clinical recommendations to providers and nursing staff.
  • Ability to supervise and oversee the work of pharmacy interns, technicians, and students, ensuring adherence to professional and regulatory standards.
  • Competence in participating on emergency response or code teams, including medication preparation and support during acute events.
  • Solid understanding of regulatory requirements and quality assurance principles related to chemotherapy and hospital pharmacy practice.
