Job Description
The Discharge Planner provides vital assistance and support to the Case Management team, ensuring a safe discharge plan for patients. This role involves collaborating with the treatment team and the patient to create an appropriate plan based on available resources and provider-created care goals. Responsibilities include discharge plan coordination, insurance utilization, and support for department employees.
Responsibilities
- Aid Case Management clinical team members in securing necessary post-acute care, DME, community resources, and transportation for patients' discharge needs.
- Coordinate communication with physicians, clinics, community resources, patients, and Case Managers regarding the continuum of care according to determined care plans.
- Provide timely and accurate information to payors, service partners, staff, patients, and their families.
- Support the coordination of utilization management and discharge planning functions.
- Assist in discharge planning and coordinate placement of the patient.
- Research and negotiate patient needs or other resources during the discharge process.
- Communicate with third-party payer case management and provide documentation as required.
- Collaborate with patients, families, physicians, nurses, and other healthcare professionals to facilitate post-hospitalization healthcare services.
- Assist in securing prior authorization from patient’s insurance provider for transition to a level of care as clinically appropriate.
- Discuss payer criteria and issues with clinical staff and follow up to resolve problems with payors as needed.
- Determine the need for discharge planning resources within one business day of being flagged for intervention.
- Support RN Case Manager with execution of an individualized discharge plan incorporating contributions from the multidisciplinary care team.
- Facilitate timely discharge of patients from the hospital setting.
- Seek consultation from appropriate disciplines/departments to expedite care and facilitate discharge.
- Facilitate referrals for home health care, hospice, medical equipment and supplies, and transfer to other facilities as appropriate.
- Document relevant discharge planning information in EMR according to department standards.
- Arrange or identify follow-up appointments post-discharge to prevent hospital readmissions.
- Provide motivated assistance in admissions, transfers, and discharges of patients managed by Case Management.
- Follow up with services arranged to ensure resources are obtained.
- Arrange patient transportation as directed by clinical staff.
- Advocate and respect patients’ rights and significant others’ rights.
- Consult with the social worker to identify and facilitate appropriate social service referrals.
Essential Skills
- Experience in case management, utilization review, and medical assisting.
- Knowledge of discharge planning and hospital inpatient procedures.
- Experience in care coordination and prior authorization processes.
- Certification as a Medical Assistant, LPN, or CNA.
- Basic Life Support (BLS) certification from the American Heart Association.
- Experience with EPIC systems.
- Strong creativity and problem-solving skills.
- Ability to multitask, prioritize tasks, and work with minimal supervision.
Additional Skills & Qualifications
- High School Diploma or GED.
- Experience in managed care and care management.
- Excellent communication and negotiation skills.
Work Environment
This position is based in a Medical Center in Hillsboro, OR. The schedule is flexible, offering either a Monday to Friday or Tuesday to Saturday shift, with a 5x8 PM schedule. Attire is business casual or scrubs. The assignment length is 13 weeks, with a high likelihood of extension and potential for permanent placement. The work environment supports discharge planning and long-stay management in a collaborative, multidisciplinary setting. Pay and BenefitsThe pay range for this position is $25.00 - $35.00/hr.
