Triage Licensed Practical Nurse (LPN)

This non–patient-facing triage role is perfect for an LPN who enjoys coordinating care, reviewing charts, navigating patient messages, and ensuring providers have the information they need—without traditional bedside or clinical duties.
⭐ What You’ll Do
As a Triage LPN, you will be a central point of coordination for patient care. Your responsibilities include:
- Conducting triage through patient calls, messages, and chart review.
- Collecting and documenting patient histories and essential medical information.
- Monitoring and identifying patient-reported physical or emotional changes and escalating as needed.
- Responding to medical emergencies within the clinic workflow and documenting patient information accordingly.
- Preparing patients for ongoing care plans post‑visit.
- Supporting and mentoring other LPNs as a knowledgeable resource.
- Scheduling, coordinating, and tracking patient tests, follow‑ups, and specialty appointments.
- Collaborating closely with faculty, fellows, and clinicians to streamline care delivery.
- Completing medication refills under MD/RN supervision through thorough chart review.
- Processing medication prior authorizations.
- Assisting patients with medication-related financial assistance programs.
- Performing additional administrative duties essential to smooth clinic operations.
⭐ What You Bring
- Active LPN License in Connecticut
- Ability to thrive in a fast-paced, multi-specialty environment
- EPIC experience is required
- Strong computer proficiency and comfort with digital workflows
- 1–2 years of medical office or triage experience
- Excellent organization, multitasking, and prioritization abilities
- Strong written and verbal communication skills
- Confidence in managing patient questions under physician guidance
⭐ Preferred Qualifications
- Experience in Neurology or medical-surgical nursing
- Strong telephone triage and patient-assessment skills
- Graduation from a state-approved practical nursing program
- One of the following:
- Four years of related experience (including two in the same job family at the next lower level) plus high school education
- OR two years of related experience plus an Associate degree
- OR an equivalent blend of education and experience
⭐ Work Environment
This role is based in the Neurology Clinic within the Multispecialty Building and is strictly non–patient-facing.
- Schedule options: 8:30 AM–5:00 PM or 9:00 AM–5:30 PM
- 37.5‑hour work week
- Flexible lunch options (30 or 60 minutes)
- Collaborative, supportive, and fast-paced administrative clinical setting
Job Type & Location
This is a Contract position based out of New Haven, CT.
Pay and BenefitsThe pay range for this position is $30.00 - $35.00/hr.
