Job Title: Case Worker I

Job Description

The Case Worker I works as a key member of a multidisciplinary HIV specialty healthcare team, providing comprehensive case management and support services to clients and their families. This role focuses on conducting culturally sensitive assessments, developing individualized plans of care, coordinating services and benefits, and educating clients and families about HIV/AIDS to promote adherence to treatment and improve overall quality of life.

Responsibilities

  • Conduct comprehensive, culturally sensitive assessments with clients and, when appropriate, their families or support networks to identify medical, psychosocial, and practical needs.
  • Synthesize program goals and assessment findings to create individualized plans of care for each client.
  • Arrange and coordinate service referrals for clients and families as needed, ensuring timely access to appropriate resources and support.
  • Provide ongoing monitoring of services and activities implemented, evaluating progress toward goals outlined in the plan of care.
  • Refer clients and families to public benefits programs when eligible and assist them in navigating the application process as appropriate.
  • Independently manage an assigned caseload, maintaining regular contact with enrolled clients to assess needs, provide support, and adjust care plans as needed.
  • Document all client contacts, service coordination activities, and care plan updates in client records in accordance with program guidelines and standards.
  • Educate clients and families about the basic medical and psychosocial aspects of HIV/AIDS to support understanding of the condition and optimize adherence to care.
  • Adhere to all applicable state laws and departmental rules and regulations related to HIV/AIDS programs and case management services.
  • Maintain current knowledge of medical and psychosocial developments related to HIV/AIDS and associated conditions.
  • Routinely interact and network with AIDS service organizations, healthcare providers, clinicians, and other stakeholders involved in HIV/AIDS service delivery.
  • Collaborate with case managers in other regions when clients reside outside of the program’s primary service boundaries to ensure continuity of care.
  • Perform indirect, non-clinical tasks related to the role, including compiling data, participating in required training, and attending staff and community meetings as assigned.
  • Maintain databases accurately and work closely with the data team to support reporting and program evaluation.
  • Function as an integral part of a multidisciplinary HIV specialty healthcare team, participating in scheduled case conferences to ensure coordinated care for clients and families.
  • Provide education to clients and families on HIV/AIDS, medication adherence, and safer sex practices to promote health and reduce risk.
  • Demonstrate strong customer service and professional phone etiquette when interacting with clients, families, and community partners.

Essential Skills

  • Ability to conduct comprehensive, culturally sensitive assessments with clients and their support networks.
  • Strong case management skills, including care planning, service coordination, and ongoing monitoring of client progress.
  • Knowledge of the basic medical and psychosocial aspects of HIV/AIDS and a willingness to maintain up-to-date understanding of developments in this field.
  • Ability to educate clients and families about HIV/AIDS, medication adherence, and safer sex practices in a clear, respectful, and supportive manner.
  • Familiarity with public benefits programs and the ability to refer clients and families to appropriate resources.
  • Strong organizational skills with the ability to independently manage a caseload and prioritize tasks effectively.
  • Proficiency in documenting client interactions and service coordination activities in client records and databases according to program guidelines.
  • Effective communication skills, including professional phone etiquette and the ability to interact with clients, families, and multidisciplinary team members.
  • Customer service skills with a client-centered approach to care.
  • Ability to work collaboratively within a multidisciplinary healthcare team and participate actively in case conferences.
  • Commitment to adhering to state laws and departmental regulations governing HIV/AIDS programs.

Additional Skills & Qualifications

  • Bachelor’s degree in a health-related field such as social work, counseling, sociology, or psychology.
  • Applicants with a bachelor’s degree in another field may qualify with sufficient case management experience.
  • Experience working in healthcare, social services, or HIV/AIDS-related programs is beneficial.
  • Comfort working with diverse populations and engaging with families and support networks.
  • Ability to compile and interpret data and collaborate with a data team to support program reporting and evaluation.
  • Experience participating in staff meetings, community meetings, and ongoing training activities.

Work Environment

This is a full-time, weekday position with a standard schedule of Monday through Friday, 8:00 a.m. to 5:00 p.m. The Case Worker I works in a multidisciplinary HIV specialty healthcare setting, collaborating closely with clinicians, providers, AIDS service organizations, and other community partners. The role involves a mix of direct client interaction, coordination with external agencies, documentation in client records and databases, and participation in team meetings and case conferences. The environment emphasizes collaboration, continuous learning about HIV/AIDS care, and a client-centered approach to service delivery.

Job Type & Location

This is a Contract to Hire position based out of St. Louis, MO.

Pay and Benefits

The pay range for this position is $22.00 - $25.00/hr.
