Ace Primary Home care is currently seeking highly motivated individual that is responsible for managing the placement of attendants in clients' homes, addressing client concerns, and ensuring high-quality service delivery within the Personal Home Care (PHC) program. Two years of PHC experience as a coordinator a must. This role requires strong communication skills and the ability to maintain positive relationships with clients and referral sources while adhering to program regulations and quality assurance standards. Experience as a PHC Coordinator is a must.
Responsibilities
- Recruiting and placing attendants in clients' homes.
- Addressing client concerns and inquiries.
- Communicating with clients, referral sources, and doctor's offices via phone and in writing.
- Performing data entry tasks related to client and program information.
- Problem-solving to address issues that arise within the program.
- Maintaining positive working relationships with referral sources.
- Providing outstanding customer service to clients and their families.
- Monitoring and tracking program authorizations.
- Ensuring timely completion of paperwork and compliance with program regulations.
Requirements
- Two years of PHC experience as a coordinator a must.
- Excellent written and verbal communication skills.
- Positive attitude and ability to work independently.
- Compassion for working with the aged and disabled.
- Verifiable work history with good references.
