- Provide skilled physical therapy services in patients’ homes under the supervision of a Physical Therapist
- Implement individualized treatment plans to improve mobility, strength, balance, and function
- Assist patients with therapeutic exercises and functional mobility training
- Educate patients and caregivers on safe movement and home exercise programs
- Monitor and report patient progress
- Document visits timely and accurately
- Collaborate with the interdisciplinary team to ensure coordinated care
- Active Florida PTA License
- Degree from an accredited PTA program
- Current CPR Certification
- Valid driver’s license & reliable transportation
- Strong clinical and communication skills
- Home health experience preferred
- Medicare experience preferred
