Key Responsibilities
- Provide safe and effective nursing care based on individualized Plan of Care
- Administer medications, wound care, infusions, catheter care, and other nursing tasks as required
- Communicate promptly any changes in patient medical status or emergencies to the team for effective case management
- Assist with patient transfers, positioning, and mobility needs
- Maintain accurate documentation in the electronic health record
- Collaborate with patients, families, and interdisciplinary team members
- Demonstrate empathy, attention to detail, and prioritize tasks effectively
- Current RN License – State of Florida
- Experience in Medicare Home Health
- Wound care and infusion skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively and provide excellent customer service
- Proficiency with electronic health records for documentation and communication
- 1 year of Home Health experience preferred
- WellSky/Kinnser experience is a plus
