Activa Home Health, An AccordCare Company is seeking experienced and compassionate Field Licensed Practical Nurses, LPNs to provide per diem home health visits throughout Hobe Sound, Stuart, Jensen, Tequesta, Indiantown and the surrounding areas.
We are a licensed, Medicare-certified home health agency committed to delivering exceptional patient care while supporting our nurses with flexibility, teamwork, and strong clinical leadership.
About the RoleAs a Field LPN, you will provide skilled, patient-centered care in the home setting, following individualized Plans of Care under the supervision of a Registered Nurse. Your work will help patients remain safe, comfortable, and independent in their homes.
Experience with WellSky/Kinnser and home health documentation is a plus!
Key Responsibilities-
Provide safe, effective nursing care per the individualized Plan of Care

Communicate promptly with the clinical team regarding changes in patient condition or emergencies

Perform skilled nursing services, including:

Medication administration
-
Wound care
-
Infusion therapy
-
Catheter care
-
Safe transferring and positioning of patients

Collaborate with the RN Supervisor and interdisciplinary care team
-
Demonstrate empathy, professionalism, and attention to detail at every visit

Active Florida LPN license
-
Wound care experience required
-
IV certification preferred
-
Minimum 1 year of home health experience preferred

Strong communication, teamwork, and customer service skills

Proficiency with electronic medical records (EMR), preferably WellSky/Kinnser
