๐ฉบ Key Responsibilities
- Deliver care in accordance with the patientโs individualized Plan of Care under RN supervision.
- Provide prompt communication of changes in patient condition/emergencies to the team.
- Perform skilled nursing tasks within LPN scope of practice, which may include:
- Medication administration
- Wound care
- Infusion therapy
- Transferring & positioning patients
- Catheter care
- Communicate effectively with patients, families, and interdisciplinary team members.
- Demonstrate empathy, professionalism, and attention to detail while prioritizing patient needs.
- Current LPN license in the State of Florida (required)
- IV Certification (required)
- Strong wound care skills
- 1 year of home health experience preferred
- Excellent communication, teamwork, and collaboration skills
- Proficiency with electronic health records (EHR)
