Position Summary
The Program Coordinator, Maternal Child Health (MCH) Male Involvement is a grant-funded role responsible for program coordination and administrative oversight of Westside Healthy Start male involvement activities. This position is responsible for coordinating the functions related to program implementation, data collection, and reporting. The position provides education, referrals, and support to expectant fathers and new parents in individual and group settings.
This position requires travel to the following ACCESS health center locations - Austin, Humboldt Park, Madison, Sinai, Warren (storage), Westside.
Core Job Responsibilities
- Coordinates the development, design, implementation, and day-to-day operations of male involvement programming at Westside Healthy Start sites
- Engages assigned or referred father/male participants in Westside Healthy Start male involvement case management curriculum across all Westside Healthy Start sites, including, assessing participant needs, developing service plans, and connecting participants to appropriate community resources
- Collects and manages program data and supports the development and maintenance of reporting tools for submission to internal and external venues
- Implements and facilitates male involvement group education sessions in accordance with grant requirements
- Provides follow-up support, including call-backs and ongoing engagement to promote sustained participation in male involvement activities
- Establishes and maintains external partnerships to support referrals and resource connections for program participants (e.g., employment opportunities, community services)
- Develops and coordinates engagement activities that promote father involvement, co-parenting and child development activities
- Prepares and distributes monthly reports summarizing program activities, participation and outcomes in accordance with grant and leadership requirements
- Collaborates with Westside Healthy Start Community Health Specialists at each Westside Health Start site to ensure co-parenting activities are successful.
- Other duties as assigned.
Requirements/Preferences
- Bachelor’s degree in a health-related field (social work, psychology, counseling, rehabilitation, sociology, or other human service field) preferred
- Minimum 2 years of working in a community based or public health setting.
- Minimum 2 years of case management experience in a public or private social service program or health care setting.
- Intermediate proficiency with Microsoft Office products and database applications (i.e. Redcap, Electronic medical record).
- 2 years of project management experience in working with high-risk populations preferred.
ACCESS is a Network of Federally Qualified Health Centers treating patients on the frontlines of community-based health care. Depending on position applied/being recruited for, candidates may be required to be vaccinated against communicable diseases and provide supporting documentation proving that they are properly vaccinated, or apply for religious and/or medical vaccination exemption as a part of the application process.
The pay ranges provided represent the minimum to mid-range for positions. Actual compensation will be determined based on a combination of factors including years of experience, educational background, market conditions, and available grant funding.
Salary : $20 - $24
