Responsibilities
As an Integrated Health Manager, you will lead care delivery and clinical operations transformations that improve throughput, utilization, and frontline performance for providers and payers. Your primary responsibilities may include:
- Design and scale preventive, chronic, and complex care programs to close care gaps
- Lead care coordination and transitions‑of‑care initiatives across payer, provider, and community settings
- Support value‑based care strategy, operating model design, and performance management
- As needed, could be up to 80%

- Minimum 5 years of consulting experience with healthcare payer and/or provider clients
- Minimum of 2 years of experience in population health, care management, or value‑based care programs
- Bachelor's Degree

- Exposure to Medicaid, Medicare Advantage, or ACO operating models
- Background in health equity, community‑based care, or behavioral health integration
- Strong understanding of longitudinal and cross‑sector care models
