Essential Functions:
- Coordinate daily clinical operations at the nurse’s station
- Serve as a point of contact for staff, residents, families, and providers
- Monitor and document changes in residents’ conditions
- Ensure timely follow-up on physician orders and lab results
- Support floor staff with clinical questions and resident needs
- Maintain accurate and up-to-date medical records
- Assist with admissions, discharges, and transfers
- Communicate effectively with interdisciplinary team members
- Help ensure compliance with facility policies and state regulations
- Provide compassionate care and maintain a resident-centered approach
- Active LPN or RN license in Pennsylvania
- Long-term care or skilled nursing experience preferred
- Strong organizational and communication skills
