Job Summary
We are seeking a Pre-Admissions Coordinator to join our team at Acara Healthcare. In this role, you will be pivotal in managing the pre-admissions process for our in-home health care services. Your contributions will directly support our mission of providing exceptional care to our patients across Central and South Texas.
Key Responsibilities
- Patient Intake & Assessment: Review patient referrals and medical records to assess eligibility for home health services. Gather clinical information and collaborate with physicians to determine appropriate care plans. Facilitate the referral intake process and coordinate care with the interdisciplinary team, the patient, their family, and the referral source.
- Preadmission Process: Ensure all required documentation, such as physician orders, patient medical history, and consent forms, are accurately completed and submitted prior to patient admission.
- Insurance Verification & Authorization: Work with the insurance department to verify patient insurance coverage and secure necessary pre-authorization for home health services.
- Patient & Family Communication: Act as the primary contact for patients and their families, educating them on home health services, explaining the admission process, and answering any questions they may have. Conduct Welcome Calls.
- Documentation & Reporting: Maintain accurate records of preadmission activities, including clinical assessments, communication, and coordination efforts.
- Care Coordination: Act as a liaison between Acara and the community regarding community and customer needs.
- Clinical Support: Provide guidance to the clinical team on patient care, addressing any questions or concerns related to the care plan or patient progress with regard to the admission and re-admission of the patient.
- Quality Assurance: Monitor the quality of patient care coordination, ensuring that clinical services meet regulatory requirements and company standards. Advocate for the patient while ensuring proper communication between all parties involved.
- Clinical Education: Support the patient and their family in understanding their care plan and providing clinical education on managing the patient’s condition at home. Develop and promote home health programs and services, while maintaining the standards of practice consistent with quality healthcare and maximizing human, financial, and equipment resources.
- Clinical Documentation: Ensure timely and accurate clinical documentation, communicating any changes in the patient’s condition or care needs to the clinical team that will affect the completion of the admission.
Qualifications
Education:
- Graduate of an accredited school of vocational/practical nursing.
- Graduate of an accredited college/university, preferred
- Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. Current driver’s license.
Experience:
- One year of experience as a licensed vocational/practical nurse
- Home Health experience preferred.
Skills:
- Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
- The ability to establish and maintain effective working relationships with all segments of the staff, the lay and professional public, the Board of Directors, and the Medical Director.
