Community Outreach Worker

Position Summary:
We are seeking a compassionate and motivated Community Outreach Specialist to join our team. This part-time position is responsible for traveling throughout Acadia Parish to assess individuals aged 60 and older for eligibility and need of our services. The ideal candidate is personable, organized, and committed to supporting the well-being of older adults in the community.

Key Responsibilities:

• Travel within Acadia Parish to meet with clients aged 60 in their homes or community settings
• Conduct assessments to determine client needs and eligibility for services
• Provide clear information about available programs and resources
• Maintain accurate and timely documentation of client interactions and assessments
• Build and maintain positive relationships with clients, families, and community partners
• Communicate effectively with internal team members regarding client needs and referrals
• Ensure confidentiality and compliance with all organizational policies and procedures

Qualifications:

• High school diploma or equivalent required; associate or bachelor’s degree in social services, healthcare, or a related field preferred
• Experience working with older adults or in community outreach, social services, or healthcare is a plus
• Strong interpersonal and communication skills
• Ability to work independently and manage time effectively
• Valid driver’s license and reliable transportation required
• Comfortable with frequent local travel

Compensation & Benefits:

• Part-time schedule (25 hours per week)
• Mileage reimbursement at $0.62 per mile
• Meaningful work serving the senior community

Work Environment:
This position requires regular travel throughout Acadia Parish and involves meeting clients in various settings. Flexibility, professionalism, and empathy are essential.
