What are the responsibilities and job description for the QA RN Home Health position at Acacia Health (Palliative, Home Health, & Hospice)?
This is a contract position expected to run through December 31, 2025, with potential for extension based on organizational needs.

- Conduct comprehensive and targeted chart audits
- Collaborate with nursing supervisors to ensure accurate and compliant documentation.
- Develop and oversee processes to measure quality, safety, risk, infection control, and patient outcomes.
- Analyze clinical records and compile data trends for reporting.
- Ensure adherence to Medicare, Joint Commission, and other regulatory standards.
- Serve as a resource for clinical supervisors and staff on regulations and best practices.
- Valid California RN License
- Bachelor's degree in nursing
- 3 to 5 year’s clinical nursing experience. Home Health nursing is preferred.
- Excellent understanding of performance improvement, quality assessment, and utilization management.
- Is selfdirected and able to work with minimal supervision.
- Has excellent coordination and communication skills.
- Detail oriented and meticulous.
- Home Health QA experience (HCHB)
- OASIS export experience
Salary : $50 - $54
