Essential Job Functions/Responsibilities:
Pay: $56.00 - $140.00 per visit

Essential Job Functions/Responsibilities:
- Conduct assessments, treatment planning, evaluation of the response to care and readjustment of the treatment plan as needed
- Plan and provide community education to patients, families and peers
- Completes accurate and timely documentation of patient care treatments in EHR (i.e., HomecareHomeBase)
- Determine a PT treatment plan
- Corona and surrounding cities.
- CA Physical Therapy License
- 2 years of physical therapy work experience
- American Heart Association Basic Life Support (BLS) certification
Pay: $56.00 - $140.00 per visit
