What are the responsibilities and job description for the UAS Registered Nurse position at AC Employment?
Responsibilities
- Conducting Admission Assessments for residents prior to admission via UAS to ensure is clinically appropriate for Medicaid Housing. Ensure Mental Health Evaluation is complete and signed by Authorized Practitioner prior to admission. Assessment may be done in house within facility or at home/ other location in resident surroundings. Confirm and verify all medications, dosage, ability to manage. Confirm resident’s appropriateness with Activities of Daily Living.
- Discuss with clinical team on appropriateness and begin to place care plan objectives and needs of the resident prior to admission.
- Completes UAS - New York Assessment instrument as needed based on the NYS DOH regulations to ensure facility remains in compliance; Upon Admission, Change in Condition, Readmission, 6 months and Annual. Forwards UAS results to Financial Team to ensure proper billing.
- Work with interdisciplinary team to develop appropriate plan of care for resident and adjust as needs of resident change. Keep up to date and communicate to team as appropriate.
- Participates in semi annual family/ resident meetings regarding results of UAS and ensure that the family/ resident are in agreement with plan of care.
- Understand and assess environmental assessments and accurate depict the resident needs and also services resident receives from both a clinical and non clinical perspective.
- Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities.
- Conducts chart audits as requested if time permitting. Report findings and adjustments to the QAPI team Quarterly.
Qualifications
- Current NY State Registered Nurse License.
- Min 1 year of UAS assessment experience preferred or willing to obtain proper UAS certification and training. Applicants with MDS experience will have an easy transition to the UAS platform.
- Excellent Communication Skills with various care members both orally and written ensuring calendar of assessments and needs of the facility are met.
- Bilingual Spanish a plus.
- Working knowledge of NYS Medicaid funding Assisted Living, Senior Housing or Community Medicaid.

- Bachelor’s Degree in Nursing
