Licensed Practical Nurse (LPN) – Pediatric Private Duty Home Care

Job Summary

We are seeking a compassionate, dependable, and highly skilled Licensed Practical Nurse (LPN) to join our pediatric home health team. The ideal candidate has experience caring for medically fragile children and is confident providing skilled nursing services in the home, school, and daycare settings. This position requires excellent clinical judgment, strong communication skills, and a commitment to delivering exceptional, family-centered care.

Responsibilities

  • Provide one-on-one skilled nursing care in the patient's home, school, or daycare setting.
  • Administer prescribed medications via oral, enteral (G-tube), or other approved routes.
  • Perform comprehensive nursing assessments and monitor changes in patient condition.
  • Provide continuous seizure monitoring, recognize seizure activity, implement seizure protocols, and respond appropriately during emergencies.
  • Monitor and manage diabetes care, including:
  • Blood glucose monitoring
  • Dexcom Continuous Glucose Monitor (CGM) management
  • Sliding-scale insulin administration
  • Recognition and treatment of hypo/hyperglycemia
  • Perform and manage G-tube and NG-tube feedings, medication administration, tube care, troubleshooting, and site assessments.
  • Provide tracheostomy, oxygen, pulse oximetry, and suctioning care when applicable.
  • Monitor vital signs and accurately document all assessments and interventions.
  • Assist patients with Activities of Daily Living (ADLs) while promoting independence whenever possible.
  • Follow individualized physician orders, nursing care plans, and emergency action plans.
  • Promptly communicate changes in patient status to physicians, parents/guardians, supervisors, and the interdisciplinary healthcare team.
  • Provide caregiver education and reinforce patient-specific treatment plans.
  • Maintain a safe, clean, and therapeutic environment for each patient.

Preferred Clinical Experience

Experience caring for pediatric patients with one or more of the following is strongly preferred:

  • Seizure disorders/Epilepsy
  • Diabetes management (Dexcom, insulin administration, sliding scales)
  • Gastrostomy Tubes (G-tubes)
  • Nasogastric Tubes (NG-tubes)
  • Enteral feeding pumps
  • Developmental disabilities
  • Autism Spectrum Disorder
  • Cerebral Palsy
  • Genetic disorders
  • Neuromuscular disorders
  • Failure to thrive
  • Prematurity
  • Respiratory disorders
  • Oxygen therapy
  • Tracheostomy care (preferred)
  • Suctioning (preferred)
  • Ventilator experience (preferred)
  • Medication administration through enteral tubes
  • Medication management for medically complex children

Skills & Competencies

  • Strong pediatric assessment skills
  • Skilled in seizure recognition and intervention
  • Proficient in blood glucose monitoring and diabetes management
  • Experience using Dexcom Continuous Glucose Monitoring systems
  • Knowledge of insulin administration and sliding-scale protocols
  • Competency in G-tube and NG-tube care and feedings
  • Ability to identify changes in patient condition and intervene appropriately
  • Excellent critical thinking and clinical decision-making skills
  • Outstanding communication and family education abilities
  • Ability to work independently while collaborating effectively with physicians, therapists, case managers, and caregivers
  • Compassionate, patient-focused, and dependable

Documentation & Compliance

  • Complete timely and accurate nursing documentation using Alora and HHA Exchange.
  • Maintain compliance with physician orders, agency policies, state regulations, and Medicaid documentation requirements.
  • Participate in ongoing education and maintain all required licensure and certifications.

Qualifications

  • Current Licensed Practical Nurse (LPN) license in good standing.
  • Current CPR/BLS Certification.
  • Pediatric home health experience preferred.
  • Experience with medically fragile or special needs children preferred.
  • Reliable transportation.
  • Excellent attendance, punctuality, and professionalism.

Job Types: Full-time, Part-time, PRN

Pay: $35.00 - $38.00 per hour
