Referral/Intake Coordinator

AtĀ Absolute Home Health and Hospice, our patients come first. We are seeking a compassionate, organized, and detail-orientedĀ Referral/Intake CoordinatorĀ to join our team. This role is essential to ensuring a smooth and welcoming experience for patients, families, and referral partners from the very first point of contact.
As our Referral/Intake Coordinator, you will manage all aspects of the patient intake process, including insurance verification and authorization, coordinating admissions, maintaining relationships with referral sources, and supporting seamless transitions of care.
Key Responsibilities
- Coordinate all aspects of the patient referral and intake process
- Establish and maintain positive relationships with referral sources, patients, families, and healthcare partners
- Verify insurance benefits and obtain required authorizations
- Ensure compliance with state, federal, and CHAP referral and intake requirements
- Coordinate patient admissions in accordance with agency policies and procedures
- Facilitate communication between patients, providers, and multidisciplinary care teams
- Track and report referral trends
- Accurately enter and maintain patient information within electronic systems
- Monitor documentation and notify leadership of missing or overdue records
- Maintain patient records and filing systems
- Answer incoming telephone calls professionally and document communications appropriately
- Participate in quality improvement initiatives
- Perform other duties as assigned
Qualifications
- High School Diploma or GED required
- Minimum of one (1) year of healthcare experience, preferably in Home Health Care
- Strong communication and customer service skills
- Knowledge of healthcare regulations, CHAP standards, insurance verification, and private payer practices preferred
- Excellent organizational, problem-solving, and time management skills
- Ability to work independently while collaborating effectively with the healthcare team
