Job Overview
We are seeking an energetic and dedicated Lead Case Manager to drive excellence in patient care coordination and team leadership. Under the supervision of the Clinical Director, the Lead Case Manager is responsible for overseeing the Case Management (and Aftercare Planning) team(s) in the services of providing education and support to clients, families, and the community. The Lead Case Manager will be responsible for ensuring data accuracy, chart compliance with daily audits, reporting status of insurance needs and payments collected, completing all team individual/team supervisions, and training team members. The Lead Case Manager will act as the primary point of contact between multi-disciplinary teams and will report daily/weekly to the Clinical Director.
Essential Responsibilities
- Job Type: Full Time; onsite: hours determined by Facility
- Conduct comprehensive psychosocial and biopsychosocial assessments.
- Ability to maintain a caseload based on current census needs or a caseload as mandated by state regulatory guidelines and supervise Case Managers/Discharge Planners.
- Provide frontline clinical and programmatic supervision to case management staff, including regularly scheduled individual supervision, case consultation, skill development, and support for ethical and professional decision-making; maintain supervision documentation and communicate relevant issues to the Clinical Director.
- Maintain oversight on developing and communicating the client discharge/transfer plans to assist in transition to appropriate care.
- Ensuring the case management team have the appropriate releases of information signed and providing appropriate case consultation and coordination with referral sources.
- Ensuring case managers document all services, assessments, reassessments, referrals and follow-up in the client's medical record, providing appropriate signatures and dating of such entries.
- Attends staff meetings and weekly department meetings as pertinent to job function and responsibility.
- Assists with transportation requests as needed in collaboration with facility transportation team.
- Overseeing the daily operations of the case management department and communicating any pertinent information to the leadership teams.
- Train all case managers in job duties and responsibilities that meet our standards of client care.
- Collaborate with the clinical director to schedule team training that will enhance the skillset and knowledge of the case management team.
- Perform supervision, individual and team, weekly for the case management team.
- Maintains standards of professional society procedures and ethical behavior and maintains client confidentiality at all times.
- Participate in performance improvement and continuous quality improvement activities.
- Represent the organization in a positive and professional manner at all times.
- Communicate the mission, ethics and goals of the organization.
- Serve as a primary clinical and programmatic resource for case management staff by responding to questions, providing guidance on complex cases, and offering leadership support in the absence of the supervisor.
- Support staff performance and program outcomes by motivating staff toward identified action plans, participating in quality improvement efforts, and assisting with data collection for quarterly and annual reporting.
- Seek consultation from supervisors, peers, or other professionals when ethical concerns, clinical conflicts, or complex situations arise.
- Perform other duties as assigned, consistent with job classification and departmental needs.
Additional Requirements:
- Minimum of three years of experience working in the behavioral health treatment field.
- Preferred minimum of 4 years’ experience working within Case Management role.
- Bachelor's (Masters preferred) degree in behavioral health related field preferred.
- Demonstrated computer proficiency.
- Prior experience working at a healthcare setting.
- Grasp of basic computer knowledge.
- Strong written/verbal communication skills.
- General knowledge of insurance terms, benefits and coverage preferred.
- Ability to assess, plan, and deliver services to individuals in need of substance use treatment and metal health services
- Ability to establish rapport, systematically gather data.
- Participation in continuing professional development.
- Ability to identify client needs that cannot be met by counselor or agency and assists clients to utilize the support systems and community resources available.
- Demonstrated knowledge of accepted principles or client record management.

- Ability to sit, stand, and walk for extended periods.
- Ability to communicate effectively with clients, families, and healthcare professionals.
- Ability to occasionally lift up to 20 pounds.
- Ability to respond appropriately in crisis situations.
Pay: $55,000.00 - $60,000.00 per year
