We are looking for an experienced Licensed Practical Nurse to join our team.
This position is responsible for the physical, emotional and social well-being of patients and coordinating with health care management.
The ideal candidate will have a background in long term care and experience working with a variety of patients in a medical environment.
This individual must be passionate about providing the best possible care for our patients and will work well with a team, quickly identifying alternate solutions when issues arise.
Responsibilities:
- Provide medical treatments and care to residents and patients.
- Meet the physical, social, and emotional needs of residents.
- Improve residents' quality of life.
- Monitor residents' medical conditions, vital signs, intake and output statistics, and general health patterns.
- Work with other nursing professionals to ensure an overall quality of care is provided.
- Work with families to help them understand the needs of their loved ones.
