What You'll Do

As a Community Inclusion Specialist, you will:

• Support adults with developmental disabilities through community-based activities, outings, and individualized programming
• Implement Individual Support Plans (ISPs) and person-centered goals
• Facilitate meaningful recreational, educational, vocational, and social activities
• Promote independence, self-advocacy, and active participation in the community
• Provide transportation to community activities, events, and appointments
• Complete required documentation, progress notes, and activity records
• Manage outing budgets and activity-related expenses as needed
• Ensure participant health, safety, and well-being, including providing basic first aid when necessary

QualificationsRequired

• High School Diploma or GED equivalent
• Minimum of three (3) years of experience supporting adults with developmental disabilities
• Valid driver's license and reliable personal vehicle
• Basic computer proficiency
• Strong communication and interpersonal skills
• Ability to lift up to 50 pounds
• Ability and willingness to safely transport individuals to appointments, activities, and community-based destinations using a personal vehicle.
  
  

Preferred

• Bachelor's Degree in Human Services or a related field
• Experience facilitating community-based programming and activities
• Knowledge of person-centered planning and community inclusion practices
  
  

Schedule

• Full-Time
• Community-Based Position
• Flexible scheduling based on participant needs and program activities
• Some evening and weekend availability may be required
  
  

Physical Requirements

• Ability to lift up to 50 pounds
• Ability to stand, walk, bend, and accompany individuals in community settings
• Ability to safely transport participants using a personal vehicle
• Ability to perform essential job functions with or without reasonable accommodation
