What You'll Do
As a Community Inclusion Specialist, you will:
- Support adults with developmental disabilities through community-based activities, outings, and individualized programming
- Implement Individual Support Plans (ISPs) and person-centered goals
- Facilitate meaningful recreational, educational, vocational, and social activities
- Promote independence, self-advocacy, and active participation in the community
- Provide transportation to community activities, events, and appointments
- Complete required documentation, progress notes, and activity records
- Manage outing budgets and activity-related expenses as needed
- Ensure participant health, safety, and well-being, including providing basic first aid when necessary

QualificationsRequired
- High School Diploma or GED equivalent
- Minimum of three (3) years of experience supporting adults with developmental disabilities
- Valid driver's license and reliable personal vehicle
- Basic computer proficiency
- Strong communication and interpersonal skills
- Ability to lift up to 50 pounds
- Ability and willingness to safely transport individuals to appointments, activities, and community-based destinations using a personal vehicle.
Preferred
- Bachelor's Degree in Human Services or a related field
- Experience facilitating community-based programming and activities
- Knowledge of person-centered planning and community inclusion practices
Schedule
- Full-Time
- Community-Based Position
- Flexible scheduling based on participant needs and program activities
- Some evening and weekend availability may be required
Physical Requirements
- Ability to lift up to 50 pounds
- Ability to stand, walk, bend, and accompany individuals in community settings
- Ability to safely transport participants using a personal vehicle
- Ability to perform essential job functions with or without reasonable accommodation
