Pay Range: $40.00 - $50.00 / hour (Determined by experience, clinical specialty, and qualifications)

Above All Talent Solutions is conducting a search for a full-time Physical Therapist to join a collaborative rehabilitation team serving adult and geriatric residents in a skilled nursing, long-term care, and post-acute rehabilitation setting.

Position Overview

In this role, you will evaluate residents, develop individualized physical therapy plans of care, and provide evidence-based interventions that support independence, safety, and quality of life. You will be a key member of an interdisciplinary rehab team, helping residents improve mobility, strength, balance, and pain management to maximize safety, independence, and quality of life. You will collaborate closely with Occupational Therapy, Speech-Language Pathology, nursing, providers, residents, families, and the broader interdisciplinary care team to support positive clinical outcomes and person-centered care.

Responsibilities

• Evaluate, diagnose, and treat adult and geriatric residents with mobility, strength, balance, gait, and pain impairments in a SNF/LTC environment.
• Develop and implement individualized, goal-driven treatment plans in collaboration with nursing, OT, SLP, and medical staff.
• Complete timely, accurate documentation of evaluations, daily treatments, progress notes, and discharge plans per facility, payer, and regulatory standards.
• Educate residents, families, and staff on therapeutic exercises, mobility aids, fall prevention, and home safety.
• Participate in care conferences, QI initiatives, and program development to support strong clinical and operational outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current, unrestricted Physical Therapist license, or eligibility to obtain state licensure before the start date.
• New graduates are encouraged to apply; relevant clinical fieldwork or prior experience in skilled nursing, long-term care, post-acute rehabilitation, inpatient rehabilitation, or acute care is a plus.
• Experience supervising or collaborating with Physical Therapy Assistants is preferred.
• Strong evaluation, clinical-reasoning, documentation, organization, communication, and interdisciplinary-collaboration skills.
• A compassionate, resident-centered, outcomes-focused approach to care.
