Above All Talent Solutions is conducting a search for a full-time Physical Therapist to join a collaborative home health rehabilitation team serving adult and geriatric patients in Brockton, Massachusetts, and surrounding communities.

Position Overview

In this role, you will evaluate patients in their home environment, develop individualized physical therapy plans of care, and deliver evidence-based interventions that support mobility, safety, and quality of life. As a Home Health Physical Therapist, you will play a crucial role in preventing hospital readmissions, managing home safety risks, and helping patients recover from acute illnesses, surgeries, or chronic conditions. You will collaborate closely with Occupational Therapy, Speech-Language Pathology, home health nurses, physicians, patients, and family caregivers to deliver high-quality, continuous, patient-centered care.

Responsibilities

• Conduct comprehensive home health evaluations, home safety assessments, and physical therapy treatments tailored to adult and geriatric patients in their home environments.
• Assess physical home environments for hazards, recommending adaptive equipment, assistive devices, and home modifications to ensure safe mobility and independent living.
• Complete timely, accurate documentation—including OASIS assessments, plan of care updates, daily treatment notes, and discharge summaries—in compliance with state, federal, and payer standards.
• Educate patients and family caregivers on therapeutic home exercise programs, safe transfer techniques, fall prevention strategies, and disease management.
• Manage an efficient daily visitation schedule and travel route within the assigned Brockton service territory.
• Coordinate interdisciplinary care with home health nurses, OTs, SLPs, physicians, and care managers to ensure smooth care transitions and positive patient outcomes.

Qualifications

• Education: Graduate of an accredited Physical Therapy program.
• Licensure: Current, unrestricted Physical Therapist license, or state licensure eligibility prior to start date.
• Transportation: Active, valid driver’s license, reliable personal vehicle, and current auto insurance coverage (required for field travel).
• Experience: Prior experience in home health care, post-acute rehabilitation, or outpatient care is preferred; familiarity with OASIS documentation is a strong plus. New graduates with home health fieldwork experience are welcome to apply.
• Skills: Strong clinical assessment, independent decision-making, time management, mobile EMR documentation, and interdisciplinary communication skills.
