Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories to inform personalized care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and other advanced therapies.
- Perform specialized procedures such as catheterization, tube feeding management, venipuncture, phlebotomy, and sterile dressing changes with precision and adherence to aseptic techniques.
- Manage complex cases involving chronic conditions like Alzheimer's disease, dementia, long-term care needs, hospice care, and memory care.
- Coordinate patient care activities including discharge planning, case management, and collaboration with physicians using electronic health record (EHR) systems such as Epic or Cerner.
- Provide education and health coaching to patients and their families on disease management, medication administration, nutrition, and lifestyle modifications.
- Maintain accurate medical documentation in compliance with HIPAA regulations and ensure timely updates of medical records.
- Support infection control protocols and ensure safety standards are rigorously followed during all procedures.
- Assist with triage assessments for urgent or emergent situations to prioritize patient needs effectively.
- Collaborate with multidisciplinary teams across hospital medicine, emergency medicine, ICU settings, outpatient clinics, and home & community care programs to deliver seamless patient-centered services.
- Travel within assigned community areas to deliver consistent care directly at patients’ homes or assisted living facilities.
Skills
- Proven experience with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation.
- Strong knowledge of medical terminology, anatomy and physiology.
- Skilled in advanced nursing procedures such as ventilator management, gastric lavage, dialysis support, and respiratory therapy including airway management.
- Expertise in managing patients with complex needs such as infectious disease care, acute pain management, behavioral health issues like Alzheimer’s or dementia care.
- Experience working with vulnerable populations.
- Ability to perform venipuncture, phlebotomy, catheterization (urinary or vascular), tube feeding management and medication administration with precision.
- Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times.
- Excellent patient assessment skills combined with compassionate communication tailored for diverse populations including those with disabilities or developmental challenges.
- Familiarity with medical imaging procedures when applicable; experience in home & community-based settings is a plus.
- Physical ability to perform duties requiring standing for extended periods and mobility across various environments; comfort working independently in the field.
