Home Health Field Nurse (RN / LPN)

Seeking a full-time home health field nurse to join our locally owned company. You will be an integral part of our team in ensuring that patients get top-notch care as a field nurse in the greater OKC metro. Enjoy flexible schedule and hours.

In addition to a 50% employer-funded health insurance plan, full time employees are eligible for paid time off (PTO) equal to 4 hours per pay period, 8 or 9 paid holidays annually, and a SEP-IRA retirement plan that is accessible after 6 months with a 3% company match.

Responsibilities:
- Perform nursing procedures according to agency policy and procedures; including but not limited to wound care, PICC line care, lab draws, foley catheter care, etc.
- Monitor patient progress to goals and report to RN Case Manager
- Educate patients and family members according to disease process, medications, plan of care, and home care procedures
- Educate patients and their families on healthcare management and resources
- Document patient information accurately and timely

Experience:

- Current unencumbered Oklahoma RN / LPN license
- Minimum of one year of nursing experience is required
- Previous home health experience preferred but not required
- Knowledge of Wellsky EMR preferred but not required

Skills:
- Proficient in both written and verbal communication
- Ability to assess patient status and interpret the appropriate information
- Capable of working both alone and in a team
- Possess excellent organizational and detail-oriented abilities

Salary : $60,000 - $77,000
