Director of Nursing – Home Health

Abe’s Garden Community is launching a new non-Medicare home health program designed to bring high-quality, relationship-centered care directly into the homes of older adults and individuals living with dementia. We are seeking an experienced and compassionate Director of Nursing to help build and lead this exciting new service.
This is an opportunity to shape a program from the ground up while bringing the trusted Abe’s Garden approach—person-centered care, dignity, purpose, and excellence—into the home.
What You’ll Do
- Lead and oversee all nursing and clinical operations for the home health program.
- Oversee and implement clinical policies, procedures, and standards of care.
- Recruit, train, supervise, and support nursing and clinical team members.
- Provide high-quality, individualized care that promotes safety, independence, and quality of life.
- Oversee assessments, care planning, clinical documentation, and ongoing monitoring of clients.
- Partner with families, physicians, caregivers, and other providers to coordinate exceptional care.
- Ensure compliance with applicable federal, state, and organizational requirements.
- Monitor clinical quality, outcomes, and client satisfaction and identify opportunities for continuous improvement.
- Help develop and grow the new program, establishing systems and processes that support sustainable operations.
What We’re Looking For
- Registered Nurse (RN) with a current Tennessee license.
- Bachelor’s degree in Nursing preferred.
- 3 years of nursing experience, with home health, geriatric, dementia, or community-based care experience strongly preferred.
- Previous nursing leadership or supervisory experience.
- Strong clinical judgment, communication, organizational, and relationship-building skills.
- Passion for serving older adults and supporting individuals living with dementia.
- Ability to work independently while collaborating effectively across disciplines.
