Home Health LPN – Rockford & Surrounding Area
Abcor Home Health is a family-owned agency that cares for both our patients and our team. We’re looking for a compassionate Home Health Registered Nurse to join us in delivering outstanding care where it matters most—at home.

Position Details
- Location: Rockford and nearby areas (within 30 minutes preferred)
- Training: 2 weeks in Arlington Heights (length tailored to your experience)
- Visits: In-home visits, ~30-45 minutes each. Minimum 5 visits per week.
- Experience: Home Health and nursing experience required
What You’ll Do
- Assess patients' physical, emotional, and social needs during home visits
- Create and follow individualized care plans
- Provide clinical care: wound care, IV/PICC line management, G-Tube and Foley care
- Educate patients and caregivers on medications and chronic condition management
- Collaborate with doctors, therapists, and care teams
- Monitor progress and adjust care plans as needed
We’re Looking For Someone With
- An Associate’s degree in Nursing (LPN preferred)
- Experience in Home Health, including OASIS documentation
- Ability to work independently and as part of a team
- Knowledge of Home Health guidelines and regulations
- Compassion, patience, and adaptability for diverse home settings
- Strong communication skills and a patient-first mindset
