The Director of Coordination oversees all care coordination operations for a high-volume home care agency. This senior leadership role ensures uninterrupted patient coverage, efficient caregiver utilization, regulatory compliance, and consistent performance across coordination teams.
Responsibilities
- Lead, manage, and hold Care Coordinators and Coordination Supervisors accountable for performance, responsiveness, and documentation standards
- Ensure full staffing of all authorized cases, including rapid response to callouts, emergencies, overtime limits, and difficult-to-service cases
- Oversee live-in and split cases, schedule adjustments, and overtime usage in compliance with NYS labor laws and agency policies
- Partner closely with HR, Payroll, Recruitment, Nursing, Intake, Authorizations, and Retention to resolve escalations and staffing challenges
- Ensure accurate and timely documentation of coordination actions, schedule changes, and case notes
- Address caregiver and patient complaints related to scheduling or communication and implement corrective actions
- Track key coordination metrics (unstaffed hours, overtime, response times, complaints) and report findings with improvement plans to leadership
Requirements
- 2 years of experience in home care coordination
- 5 years of leadership or management experience in home care
- Strong knowledge of NYS home care regulations and labor considerations
- Proven experience managing high-volume, fast-paced coordination teams
- Excellent communication, problem-solving, and accountability skills
- Experience in a Licensed Home Care Services Agency (LHCSA) preferred
- Experience with HHA Exchange preferred
- Experience overseeing multi-region or high-census coordination teams is a plus
