Responsibilities
- Provide direct patient care with a focus on safety, comfort, and clinical excellence, including vital signs assessment, medication administration, and physical examinations.
- Lead nursing staff by coordinating daily operations, delegating tasks, and ensuring adherence to all regulatory standards and facility policies.
- Oversee patient assessments, develop individualized care plans, and monitor progress to optimize health outcomes.
- Manage complex clinical procedures such as IV infusions, catheterization, wound care including suturing, and airway management.
- Document all patient interactions accurately using Electronic Health Record (EHR) systems like Epic or Cerner while maintaining strict compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and post-acute care services.
- Respond promptly to emergencies or acute changes in patient conditions, utilizing skills in triage and emergency medicine protocols.
- Support infection control practices and ensure compliance with long-term care regulations and safety standards.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification required.
- Demonstrated experience in critical care environments such as ICU or trauma centers is highly preferred.
- Proven background working with diverse populations including geriatrics, individuals with developmental disabilities, dementia or Alzheimer’s disease, and memory care patients.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and medical documentation standards.
- Experience with EMR/EHR systems such as Epic, Cerner or Athenahealth; proficiency in electronic health records management is essential.
- Skilled in clinical procedures including phlebotomy, injection administration, sterile technique, tube feeding, ventilator management, and first aid.
- Excellent communication skills for effective caregiver communication and patient assessment; ability to work well under pressure in fast-paced settings such as emergency room or urgent care environments.
- Knowledge of infection control protocols and long-term care regulations; experience working with individuals with disabilities or complex medical needs is a plus.
