- Coordinate transition activities for children exiting early intervention (BabyNet) into preschool special education or other community-based programs.
- Support families in understanding differences between early intervention (IFSP) and preschool special education services (IEP).
- Coordinate transition planning for students with disabilities (typically ages 1421) as part of the IEP process.
- Support development of measurable postsecondary goals and transition services aligned to student strengths, preferences, and interests.
- Participate in student-centered IEP meetings with a focus on transition outcomes.
- Assist students in accessing work-based learning experiences, internships, job shadowing, and vocational training opportunities.
- Collaborate with families and staff to prepare students for post-secondary education, employment, and independent living.
- Develop and maintain partnerships with community agencies, including early intervention providers, vocational rehabilitation, colleges, employers, and other service organizations.
- Connect families with adult service agencies and community resources.
- Collaborate with general and special education staff, related service providers, and administrators across grade levels.
- Ensure compliance with IDEA (Part C and Part B) and all state transition requirements.
- Monitor and analyze transition data, including early childhood timelines and post-school outcomes, to inform program improvement.
- Support district initiatives related to early childhood services and college/career readiness.
- Assist families by providing guidance and resources throughout all transition stages.

- High school diploma or equivalent
- Bachelor's degree in Special Education, Early Childhood Education, Counseling, Social Work, or a related field preferred.
- Experience working with children and students with disabilities across early childhood and/or school-age settings.
- Knowledge of IDEA Part C and Part B regulations, including transition requirements.
- Experience collaborating with families and community agencies.
- Strong interpersonal and communication skills
- Ability to build relationships with families, staff, and community partners
- Knowledge of child development and transition planning across age spans
- Strong organizational and case management skills
- Ability to manage timelines and ensure compliance
