What you’ll do
- Provide high-quality, evidence-based primary care to a diverse patient population.
- Engage in population health initiatives and coordinate care across settings to improve outcomes.
- Participate in teaching and professional development activities—help mentor trainees and contribute to our graduate medical education programs.

Who we’re looking for
- MD or DO from an accredited medical school.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Completion of an accredited Internal Medicine or Family Medicine residency.
- Eligible for, or able to obtain, an unrestricted New York medical license prior to employment.
- Current BLS certification.
- Experience with Epic or a comparable enterprise electronic health record.
- Cultural competency or multilingual skills that reflect eastern Brooklyn’s communities (Spanish, Haitian Creole, Bengali, or other commonly spoken languages strongly preferred).
- Interest in value-based care, population health, and care coordination.
- A desire to teach and take part in graduate medical education.
