Join a growing team delivering patient-centered primary care in skilled nursing facilities across Wyoming, Michigan. This part-time role offers flexible scheduling, six-hour daytime shifts (typically starting between 7:00–10:00 a.m.), no overnight or on-call duties, and collaborative practice with physicians and facility staff to provide high-quality, evidence-based care.
Locations
- Primary: Wyoming, MI
- Additional Michigan sites: Bad Axe, Iron River, Mt. Pleasant
- Nationwide opportunities: GA, IL, IN, MO, OH, TN, WI
Key responsibilities
- Conduct daily rounds and manage acute and chronic conditions in SNF residents
- Perform 15–20 patient visits per day on average
- Prescribe medications, develop treatment plans, and ensure continuity of care
- Collaborate with nursing and interdisciplinary teams to optimize outcomes
- Maintain compliance with clinical and regulatory standards
Schedule & work environment
- Part-time with flexible scheduling
- Six-hour facility shifts; no call or overnight coverage

Qualifications
- Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
- NP board certification: ANCC, AANPCB, PNCB, or AACN; PA certification: NCCPA
- Clinical focus: Board Certified or Board Eligible in FM, IM, Geriatrics, PM&R, or ED
- Strong clinical judgment, communication skills, and ability to work independently while engaging effectively with a multidisciplinary team
