Summary
As an Intake Coordinator, you will be responsible for managing and processing initial client or patient information, ensuring that all necessary documentation is collected, and providing support to the team. You will work closely with clients to gather personal and confidential information, verify data, and ensure that all required forms and files are complete. Your role will help maintain a seamless and efficient intake process, ensuring that clients have a positive experience from the start.
Key Responsibilities
- Client/Patient Communication: Serve as the first point of contact for clients or patients, answering inquiries and guiding them through the intake process.
- Data Entry & Documentation: Collect and input necessary personal, financial, and medical information (if applicable), ensuring accuracy and confidentiality.
- Eligibility Verification: Confirm eligibility for services or programs, including verifying insurance or other required documentation.
- Scheduling: Coordinate initial appointments, assessments, or meetings as needed, ensuring proper follow-up.
- Collaboration: Work with clinical, administrative, and case management teams to ensure smooth transitions for clients through various stages of care or services.
- Confidentiality: Maintain strict confidentiality of client or patient records in compliance with HIPAA (or applicable regulations).
Qualifications
- Previous experience in intake coordination, customer service, administrative support, or related fields preferred.
- Strong organizational skills, attention to detail, and the ability to multitask effectively.
- Excellent verbal and written communication skills, with the ability to interact with clients or patients professionally and compassionately.
- Ability to maintain confidentiality and handle sensitive information appropriately.
