The Intensive Care Coordinator (ICC) is responsible for overseeing the care coordination for youth clients in the foster care system or at risk of out-of-home placement. The ICC brings together all the providers and caregivers in a client’s life to assist the client in stabilizing their symptoms and improving functioning. This position facilitates collaborative Child and Family Team (CFT) meetings and provides direct services including counseling, case management, skill building and parent support as well as cross-system coordination to promote stability, and long-term success for youth and families.

Duties and responsibilities

  • Deliver collaborative, culturally sensitive, strengths based, client centered individual/group/family counseling and rehabilitation services.
  • Provide immediate crisis intervention and stabilization for youth and their families in high stress situations.
  • Coordinate care for clients with a multidisciplinary clinical team and outside agencies.
  • Facilitate referrals to community resources and perform targeted case management services.
  • Complete all documentation per Medi-Cal, Santa Clara County and AACI standards within expected deadlines.
  • Develop effective working relationships with outside agencies and organizations.
  • Attend CFT meetings, including information gathered during these meetings into client’s treatment plan and CANS assessment.
  • Develop culturally relevant assessment and treatment plans.
  • Participate in clinical supervisions, consultations, team meetings, trainings, and after-hours rotation.
  • Licensed Clinicians only- Complete and maintain LPS 5150 Authorization certification and provide officer of the day support.
  • Complete other related duties and projects as assigned.
Qualifications:

Education & Experience:

  • Master’s degree in psychology, social work, or related field, or equivalent specialized training and experience.
  • Two years or more experience working with high-needs children and families.
  • Experience working with the Department of Social Services, Mental Health, and other county agencies preferred.

Knowledge, Skills, & Abilities:

  • Knowledge of DSM-V criteria for diagnosing behavioral health illnesses.
  • Knowledge of trauma-informed and evidenced-based practices (i.e., CBT, DBT, ACT).
  • Understanding of cultural issues, community resources and family dynamics.
  • Knowledge of legal and ethical requirements of board of licensure.
  • Knowledge of mandated reporting laws in California.
  • Knowledge of community and county resources to facilitate referrals and coordination with appropriate agencies.
  • Ability to exercise independent clinical judgment within the scope of the program responsibilities.
  • Ability to implement evidenced-based techniques to address clients’ symptomatology and functional impairments.
  • Working knowledge with Microsoft Office Suite.
  • Knowledge of or aptitude to learn Epic health record system.
  • This position requires driving. A valid California driver license, current personal auto insurance, and an MVR sufficient to maintain insurability under agency auto liability policies.
  • Verbal and written fluency in English required; Bilingual skills in a second language (particularly Spanish, Mandarin, Vietnamese) desired but not required.

  • Ability to establish and maintain effective work relationships as part of a multi-disciplinary team.

Working conditions

This job requires you to be in the community, in client’s homes, and other community partner sites. Work is often conducted in an office environment. Basic safety precautions and use of protective clothing or gear may be required. Working with clients who are seriously mentally ill, or high risk may increase exposure to secondary trauma. Driving is essential to the function of this job. The job requires travel up to 50% of the time.
