Hospice & Home Visit Clinic
Position Summary
We are seeking a compassionate and highly skilled Registered Nurse (RN) to join our team in a part-time capacity, supporting both hospice care and clinic-based home site visits. This role requires a nurse who is clinically strong, independent, and comfortable delivering care in patients’ homes.
The ideal candidate will have strong experience in wound care, IV therapy, and direct patient care, with a commitment to providing high-quality, patient-centered services.
Key Responsibilities
- Provide skilled nursing care to patients in hospice and home settings
- Perform comprehensive patient assessments and develop care plans
- Deliver and manage wound care treatments (including advanced wound care as needed)
- Initiate and maintain IV therapy, including medication administration
- Monitor patient conditions and report changes to physicians and care teams
- Educate patients and families on treatment plans, medications, and care techniques
- Maintain accurate and timely clinical documentation
- Collaborate with interdisciplinary teams to ensure continuity of care
- Ensure compliance with all clinical, regulatory, and safety standards
Qualifications
- Active Registered Nurse (RN) license in [State]
- Minimum 1–2 years of nursing experience (home health or hospice preferred)
- Strong proficiency in:
