About the Role:
Aachele Home Care and Hospice is seeking a compassionate Hospice Chaplain to
provide spiritual and emotional support to patients and families in their homes. As part
of our interdisciplinary team, you will offer counseling, prayer, and guidance tailored to
the beliefs and preferences of each patient and family.
Responsibilities
Visit patients and families in their homes to provide spiritual and emotional
support.
Collaborate with the interdisciplinary group (IDG) to ensure the patient’s spiritual
needs are included in the plan of care.
Provide grief counseling, crisis support, and bereavement care.
Document visits and care provided in the patient’s record.
Coordinate with community clergy or spiritual leaders upon request of
patients/families.
Participate in team meetings and provide input into patient/family care planning.
Requirements:
Completion of Clinical Pastoral Education (CPE).
Current CPR and First Aid certification.
Negative TB/PPD skin test result.
Successful completion of a background check.
Valid driver’s license and reliable transportation.
Preferred:
Prior hospice, hospital, or healthcare chaplaincy experience.
