Position Summary:
We are seeking a detail-oriented and compassionate Licensed Practical Nurse (LPN) to perform in-home client assessments. This role is essential in evaluating client needs, developing care plans, and ensuring appropriate service delivery in collaboration with our care team.

Key Responsibilities:
- Conduct initial and ongoing in-home client assessments
- Evaluate client health status, functional abilities, and care needs
- Develop and update individualized care plans
- Document assessments accurately and in a timely manner
- Communicate findings with caregivers, families, and interdisciplinary team members
- Ensure compliance with agency policies, state regulations, and standards of care
- Participate in quality assurance and follow-up visits as needed
Qualifications:
- Current, active LPN license in Georgia
- Minimum [1–2] years of nursing experience (home care experience preferred)
- Strong assessment and clinical documentation skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
- Valid driver’s license and reliable transportation
