• 2-week rotating schedule: Week 1 — Monday, Tuesday, Wednesday, Friday, Saturday | Week 2 — Sunday, Monday, Wednesday, Thursday, Saturday
Key Responsibilities
- Administer medications and treatments per physician orders and facility policy
- Perform wound care, catheter care, and other skilled nursing procedures
- Complete nursing assessments and contribute to care plan updates
- Document accurately in the EMR and communicate patient status changes
- Supervise CNA/GNA staff as assigned
- Support infection control, regulatory compliance, and quality care initiatives
Requirements
- Active state LPN/LVN licence (or compact licence eligibility)
- CPR/BLS certification current
- Minimum 1 year of LPN experience in a LTC/SNF or similar setting
- Proficiency with EMR documentation and medication administration
- Strong communication skills and ability to work within a multidisciplinary team
