Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews, to determine health status and needs.
- Develop and implement individualized treatment plans, incorporating evidence-based practices and current clinical guidelines.
- Administer medications and injections, including IV infusions and botulinum toxin treatments, ensuring safe and effective delivery.
- Coordinate with multidisciplinary teams for discharge planning, case management, utilization review, and post-acute care transitions.
- Provide patient education on health coaching topics such as nutrition, medication adherence, memory care (Alzheimer’s/dementia), family planning, and lifestyle modifications.
- Maintain compliance with HIPAA regulations and infection control protocols to ensure patient confidentiality and safety.
- Assist in clinical research initiatives or quality improvement projects to advance healthcare practices within the organization.
Experience
- Proven experience as a Nurse Practitioner in diverse clinical environments.
- Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth, or similar platforms.
- Skilled in diagnostic evaluation utilizing laboratory experience or medical imaging techniques; knowledge of sonography is advantageous.
- Adept at working with diverse populations, including children (pediatric care), seniors (geriatrics), patients with developmental disabilities or memory impairments.
