About the Role
We are expanding our operations across Southern Utah and are looking for a driven, relationship-focused Community Liaison Representative to join our team. In this role, you will be the face of A Plus Healthcare at Home — building and maintaining strategic relationships with hospitals, physicians, skilled nursing facilities, and community organizations throughout Salt Lake City, Lehi, Provo, Murray, West Valley, and the surrounding areas to connect patients with the exceptional home health and hospice care they deserve.
Responsibilities
- Develop and nurture strategic relationships with community hospitals, nursing homes, physicians, and healthcare organizations throughout Salt Lake City, Lehi, Provo, Murray, West Valley, and surrounding communities
- Conduct on-site meetings and presentations with physicians, hospital staff, and medical facility professionals to promote home health and hospice services
- Collaborate with clinical field teams and management to develop and execute a strategic marketing plan aligned with agency growth goals
- Participate in weekly support team meetings and provide ongoing progress updates throughout the week
- Work to drive quality patient outcomes through strong partnerships with referral sources and community stakeholders
- Identify and cultivate new referral opportunities within the local Southern Utah healthcare community
Required Qualifications
- Minimum 1–2 years of marketing or sales experience in home health; hospice experience highly preferred
- Established relationships with referral sources in the Salt Lake City, Lehi, Provo, Murray, West Valley, or surrounding areas a strong plus
- Experience in pharmaceutical, assisted living, skilled nursing, or other healthcare sales also considered
- Exceptional communication skills — phone, text, and email — and the ability to work both independently and collaboratively
- Outgoing, relatable, and team-oriented with strong interpersonal skills
- Valid driver's license, clean driving record, and current auto insurance required
