Utah Home Health and Hospice is expanding and seeking a compassionate Part-Time Registered Nurse to support our patients primarily in the D/W county area. Responsibilities
- Perform comprehensive nursing assessments in the home setting
- Provide and coordinate patient care according to individualized care plans
- Document visits and progress accurately and in compliance with regulations
- Offer clinical insight and support to the care team
- Utilize EMR systems and current technology to enhance patient outcomes
- Collaborate with a supportive, communicative interdisciplinary team
Qualifications
- Active Registered Nurse (RN) license in the State of Utah
- Home health or hospice experience preferred (willing to train)
- Experience with Homecare Homebase (HCHB) or similar EMRs is a plus
- Strong communication and assessment skills
- Commitment to patient confidentiality and quality care
