Job Summary:
Primary function is to facilitate the referral intake process and coordinate care with the interdisciplinary team, the patient, their family, and the referral source
Job Qualifications:
Education: Graduate of an accredited school of vocational/practical nursing.
Licensure: Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. Current driver’s license.
Experience: One year of experience as a licensed vocational/practical nurse; home health experience preferred.
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Transportation: Reliable transportation and valid auto liability insurance.
Physical and Mental Effort:
Prolonged sitting and some standing required. The occasional need to lift, pull, carry, and push items weighing up to 50 pounds.
Frequent need to stoop, kneel, and reach while accessing files. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions. Requires hand-eye coordination and manual dexterity. Requires excellent problem
solving skills
Essential Functions:
Receives and appropriately documents new referrals in a timely manner.
Participates in coordinating care with nursing supervisors, patients, and the interdisciplinary team.
Provides effective communication to patients, staff members, other healthcare professionals, and referral sources.
Responds to the Agency and patient needs in a professional and creative manner.
Demonstrates commitment, professional growth, and competency.
Assist with afterhours processing of referrals to ensure timely initiation of care.
Participates in the Agency’s sponsored in-service trainings.
Verifies patient benefits to ensure payer requirements are met prior to acceptance of the patient
Collects or assists the billing personnel in receiving patient responsible payments timely.
Ensures accurate entry, access, and retrieval of database information.
Professionally interacts with the patients, families, and referral sources to meet the needs of the patients
Works with the marketing team to ensure all viable referrals are processed timely and efficiently
Participates in the Agency’s QAPI program.
Assist with after-hours processing of referrals to ensure timely initiation of care.
Promotes the Agency’s philosophy and administrative policies to ensure quality of care.
