Qualifications and Skills:
- Current Licensed Vocational Nurse (LVN) license in the State of Texas
- Graduate of an approved school of professional nursing
- Current CPR certification preferred, but not required
- Minimum of one year of experience in an acute care setting preferred
- Experience performing heart, lung, and full physical assessments preferred
- Strong clinical judgment and critical thinking skills
Responsibilities and Duties:
- Adhere to the Texas Nursing Practice Act, Board of Nursing rules, and all applicable regulations
- Under the direction of the RN, assists in identifying the patient's physical, psychosocial, and environmental needs as evidenced by documentation.
- Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN. Provides services that are ordered by the physician as indicated in the plan of care.
- Provides patient and caregiver education
- Provides care using infection control measures that protect both staff and patient.
- Assures the continuity of care through delivery of quality patient care.
- Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner.
- Monitors assigned cases to ensure compliance with requirements of thirdparty payor.
- Documents care timely by meeting agency's standards of submission of documentation.
- Demonstrates commitment, professional growth, and competency.
- Clarify and verify questionable physician orders
- Assist with staff training and orientation
- Maintain compliance with agency policies
- Perform additional duties as assigned
